In a blender combine: bananas, vanilla extract and 1 cup milk and blend it well until it is smooth.
Add the vanilla ice cream to the banana milk.
And with the blender motor running, add the remaining milk and blend, until it is thoroughly mixed.

Very nice. the vanilla adds that little something that separates it from the fast food joint milkshakes, however make sure your blender is big enough... oops.
